This may come from:

  • increased testosterone production 
  • increased androstenedione

Even when total testosterone is normal, free testosterone may be elevated.

What happens in the skin?

Sebaceous glands contain androgen receptors.

When testosterone enters the sebaceous gland it is converted by 5-alpha reductase into the much stronger androgen:

Testosterone → DHT (dihydrotestosterone)

DHT:

  • enlarges sebaceous glands 
  • increases sebum synthesis 
  • changes the lipid composition of sebum 
  • accelerates sebocyte proliferation 

Result:

  • more oil
  • thicker oil
  • larger follicles

Sebum itself isn't the problem. The problem is too much sebum combined with abnormal keratinization.

Inside the follicle:

  • keratinocytes divide faster 
  • dead cells don't shed normally 
  • sebum accumulates 
  • the follicular opening narrows 

This creates a microcomedone. The microcomedone is the earliest acne lesion. Everything begins here.

High insulin contributes to acne through several pathways.

Insulin lowers SHBG (sex hormone-binding globulin)

Normally the liver produces SHBG, which binds testosterone. High insulin suppresses SHBG production. Less SHBG means more free testosterone = more free testosterone reaches the skin.

Insulin directly stimulates ovarian androgen production

High insulin acts on ovarian theca cells. The ovaries respond by producing even more testosterone.

This creates a vicious cycle.

Neutrophils migrate into the follicle.
