Step 1: Hyperandrogenism stimulates the sebaceous gland

The hallmark of PCOS is excess androgen activity.

This may come from:

increased testosterone production
increased androstenedione
increased DHEAS (adrenal source in some women)
decreased Sex Hormone Binding Globulin (SHBG), leaving more free testosterone.

Even when total testosterone is normal, free testosterone may be elevated.

What happens in the skin?

Sebaceous glands contain androgen receptors.

When testosterone enters the sebaceous gland it is converted by 5-alpha reductase into the much stronger androgen:

Testosterone → DHT (dihydrotestosterone)

DHT:

enlarges sebaceous glands
increases sebum synthesis
changes the lipid composition of sebum
accelerates sebocyte proliferation

Result:

more oil
thicker oil
larger follicles

Step 2: Excess oil changes follicular biology

Sebum itself isn't the problem. The problem is too much sebum combined with abnormal keratinization.

Inside the follicle:

keratinocytes divide faster
dead cells don't shed normally
sebum accumulates
the follicular opening narrows

This creates a microcomedone. The microcomedone is the earliest acne lesion. Everything begins here.

Step 3: Insulin resistance makes everything worse

Around 50–80% of women with PCOS have some degree of insulin resistance, even if they are not overweight.

High insulin contributes to acne through several pathways.

Insulin lowers SHBG (sex hormone-binding globulin)

Normally the liver produces SHBG, which binds testosterone. High insulin suppresses SHBG production. Less SHBG means more free testosterone = more free testosterone reaches the skin.

Insulin directly stimulates ovarian androgen production

High insulin acts on ovarian theca cells. The ovaries respond by producing even more testosterone.

This creates a vicious cycle.

Step 4: The follicle becomes clogged

Now the follicle contains:

excess sebum
excess keratin
limited oxygen

This is the ideal environment for Cutibacterium acnes. These bacteria normally live on healthy skin.

They become problematic when trapped inside a blocked follicle.

C. acnes metabolizes sebum and releases enzymes that break triglycerides into free fatty acids.

Neutrophils migrate into the follicle.

The follicle wall weakens and eventually ruptures.

Inflammatory material spills into the surrounding skin.

cysts
